[*] Scope of Service:

(1) Vendor needs to provide closed loop referral system services
- Seeking a web-based SaaS (Software as a Service) Closed-Loop Referral System (CLRS) that incorporates current Industry Standard tools and technology, adaptability and scalability to accommodate current trends, technological advances, and regulatory requirements
- System must integrate into the ZeOmega JIVA System and be able to integrate with company-wide interoperable member patient profile
- Authorized Users will have the ability to actively use the CLRS Solution to connect members to Domain-aligned CBOs, resources and programs
- to help Health and other Authorized Users better understand the members they are serving and to further strategies aligned with increasing member’s overall health and wellbeing.
- The application must have the ability to interface with third-party applications including clinical documentation system – JIVA, internal referral systems, and forthcoming customer relationship management (CRM) platform
- The application must support Single-Sign-On (SSO) from Provider Portal for providers and from Active Directory for staff users.
- The application must transmit all relevant data back to as part of database replication or a nightly/weekly data feed
- The application provider must demonstrate a robust disaster recovery and business continuity plan in place to safeguard against potential disruptions
(2) All the questions must be submitted no later than November 28, 2023.
